I read Neil Fiore's book, The Now Habit, which has some ideas on how to stop procrastinating. But mainly I ended up needing to go to my school's counseling service to talk out what I was feeling and why with one of the therapists there. Not sure if that's an option for you, but as a pathological procrastinator myself (hopefully recovering), I wish you luck...
